Does Comprehensive Behavioral Health (CBH)/Blair Vinson Kleiber, Ph.D. accept insurance?

CBH/Blair Vinson Kleiber, Ph.D. are not currently contracted with any insurance panels and is considered out-of-network provider.  You may contact your insurance company to determine if they will reimburse you for services provided by CBH through your out-of-network benefits. Many insurance plans do provide the opportunity to apply for reimbursement for therapy services received from out-of-network providers. Please call your insurance company prior to your first appointment and request further information regarding your out-of-network benefits. Some helpful questions to ask your insurance company:

·       I’m seeking out of network services with Comprehensive Behavioral Health/Dr. Blair Vinson Kleiber, what are the percentage of fees that are reimbursable?

·       Is there a deductible?

·       What is the maximum allowable?

·       How many sessions per year does my plan cover?

Will CBH submit my charges for out-of-network coverage?

Clients at CBH are responsible for submitting their own statements to insurance.  A monthly invoice will be provided to submit to your insurance company for reimbursement.

What are the typical psychotherapy billing codes?

·       Initial Evaluation – Billing CPT Code: 90791

·       Recurring Sessions – Billing CPT Code: 90834 (38-52 minute session) or 90837 (53-60 minute session)

·       Group/skills training sessions – Billing CPT Code: 90853

What is the payment policy?

CBH’s policy is payment at the time of service and requires a credit card to be placed on file prior to starting service with CBH. Clients are responsible for all charges incurred whether or not insurance provides coverage. CBH provides clients with a monthly billing statement for the services rendered and fees paid to submit to insurance companies for out-of-network reimbursement.

What forms of payment do you accept?

Check, Visa or Mastercard.  Clients are responsible for all fees at time of service.

What is your cancellation policy?

If it is necessary to cancel an appointment, you must cancel 48 business hours before your scheduled appointment time. You will be charged the regular fee for that session if you have not notified our office 48 business hours before the time of the appointment. All voicemails are dated and time stamped.  If your appointment is scheduled for Monday, please leave a voicemail on Friday to confirm your cancellation.

What are your hours?

We respond to calls between 9am-5pm Monday through Friday.

What are your policies regarding confidentiality?

All information you provide is confidential with a few expectations. Your provider will review a consent form that details limits to confidentiality during your first meeting.
